1.	I understand that I am volunteering at my own risk.
2.	I agree to abide by the policies and procedures presented to me during volunteer training and in accordance with instructions given in my assigned area.
3.	I understand that I am making a commitment and others are depending on me to fulfill that commitment. Should I not be able to meet my obligations I will communicate this to my supervisor as soon as possible.
4.	I understand that upon acceptance as a Pet-Evacuation Volunteer I will be credentialed for the City evacuation ONLY and must evacuate the City after the assigned volunteer tasks are completed. Credentials expire the day the disaster strikes and do not allow re-entry into the City.
